Once you have scheduled your initial appointment, we ask that you complete the relevant new patient forms to allow providers more time to focus on your medical care. Please submit these forms within 2 – 3 days of scheduling an appointment. If you have any questions please give us a call at 718-238-2100.
Our medical providers will then visit your home at the scheduled day and time. Please make sure to prepare all your medications, vitamins, home remedies, and prescriptions; copies of medical logs and test results; any hospital discharge papers and patient summaries. During the initial visit, our team will perform a comprehensive physical examination and include any necessary diagnostic and medical testing including blood work, EKGs, x-rays, ultrasounds, and ECHOs. They may make recommendations for patients to visit a specialist or for follow-up appointments.

The Doctors on Call team consists of experienced multilingual doctors, nurse practitioners, and physician assistants. All our nurse practitioners and physician assistants are board certified. They are specially trained and certified to provide comprehensive medical care, including examining and diagnosing patients and prescribing medications, in addition to performing diagnostic testing such as blood work and x-rays at home.
